It really depends what your management objectives are. I personally believe that the number of objectives that you listed would be too many for a single questionnaire. If your objective is to measure the effectiveness of the campaign, I would do a pre- and post-hoc measurement to measure the incremental effect of the campaign. What you will be measuring really depends on the objectives of the campaign itself. Was it used to just generate awareness? Was it used to reposition the company?
